Blue?

I guess didn't forget it, I don't ever recall seeing it. My earliest computing experiences go back as far as Apple IIs (green) and TRS-80s (b&w) in the late '70s and amber in the early 80s.

Maybe blue came before or after?

I don't know if it came before or after, but it was around in the early 80s. Obviously, it wasn't as popular as the green, white or amber ones.
Has anyone ever seen a red monochrome CRT?

I never have. But, I used to do level 2 tech support many years ago on this really large red/orange plasma displays that could display 4 different sessions on the same screen.
