Wisp-router gets them from Pac Wireless here in Utah (as they do most their antennas).

Here's one....Watertight, Bulkhead, Ethernet, but....no pigtail. Close to what your looking for.

I don't think I have EVER seen what You are looking for?. How about a picture or drawing?. SmartBridges uses a strange looking connector on their APPO's, and Tranzeo has a very different setup on their units too, but I get the impression You are looking for something different?
By the way, I don't have any hens teeth, but I do have some rooster lips if You need them?, HEHE!:D

Does anyone know where I can buy the RJ45 bulkhead connector (waterproof type), with the RJ45 pigtail?

They seem to be as difficult to find as hen's teeth...LOL.
