I wasn't trying to put words in your mouth. I understood what you wrote, but in your post, you also said that there may also be some other adapters that the DGL-4300 would have problems with but weren't sure. That uncertainty was enough for me not to get the DGL-4300. Of course I could have had the same problem with my new router as well, but I got lucky I guess.

You definitely should keep the router between the modem and the adapter to get your money's worth. I think what I'd do instead of replace the D-Link is replace the adapter instead with a brand/model that D-Link used to test with so you know it should work. Adapters cost less than replacing the DGL-4300. Then I'd sell the adapter on Ebay to someone who can use it to defray your costs.

I am informed offline that the LinkSys PAP2 is known to work with the 4300.

Just an FYI, I have tried this with the Linksys PAP2 and it does not work either. I am having the same issues as the original poster and have also tried all the port forwaring and DMZ with no luck. Quite frustrating to say the least. WTF is DLINK thinking???!?!

Hint in the Help pages for Special Applications:

"SIP: Allows devices and applications using VoIP (Voice over IP) to communicate across NAT. Some VoIP applications and devices have the ability to discover NAT devices and work around them. This ALG may interfere with the operation of such devices. If you are having trouble making VoIP calls, try turnig this ALG off."
