4-6mbps can be pushed over a regular ADSL line. BellSouth announced that they were going to invest $34 billion into their whole infrastructure to upgrade to ADSL2+ (25mbps). Seriously if they are going to invest all that money for only 4-6mbps that would be a huge mistake. Hopefully with that much we will be able to see 25mbps. The distance rates for ADSL2+ are very critical as for speed goes.

Because they can beat the cable companies in terms of speed but they'll be able to offer more services. As always bundling is where the money's at.
