I firmly believe this is nothing more than a bad photoshop.
Notice that all the elements in the picture are out of alignment, and the edges of the switch plate do not cast a shadow onto the wall. |

Whether or not the picture is chopped the plate is real. I've seen the same wall plate live and it wasn't at a Doctor's Office.
Anyone with a vinyl cutter can make you one.
