The answer to your questions depend partly on how DSLX and VZ handle the transfer.

DSLX offers an Express ISP Change service. However, I don't know if that applies to fiber.

If it is handled as a billing transfer of an existing account, then VZ probably wouldn't send anyone out. However, considering all the billing issues posted in the VZ forums and the way VZ ties service to a phone number (or pseudo-phone number if no VZ phone service), I think this is unlikely.

IMO, it is more likely that it will be treated as a disconnect and new service. i.e. DSLX is the customer and it would be included in their master billing. If this is the case, you would probably be subject to an ETF if you are under a contract plan with VZ. Since for billing purposes, this is a new install, then VZ might send someone out to ensure everything is working.

If you currently a coax connection to your router, I think it is unlikely that the tech would switch you to cat5 without charging you for doing so. See my post here for the likelihood of getting cat5 on a new install.

My recommendation is before you switch to DSLX:
•Run the cat5 yourself (or hire someone to do it).
•Connect it to the ONT.
•Call the FSC or post in »/forum/vzdirect to have your provisioning switched from coax to cat5.

Once you're on a cat5 connection, then go ahead and switch to DSLX. If a VZ tech does come out, simply ask that he leave you on the cat5 connection. If one does not come out, you're already on cat5, so no problem.
