Western Union here does not ask for anything, no paperwork at all. You hand them the money and they hand you the card. It is pretty much the same at all the places that sell these cards.

Cleveland, OH		 Your WU is in violation of their own TOS. You have to fill out a transaction sheet as well.

That's why they have a form that states "prepaid services" on it.
