The barrel connector on the pigtail adapter has splits in it. If you gently squeeze them inward it will tighten the connection to the modem.
Mine is one solid part, no splits.

 Grab some needle-nose pliers and just try squeezing it a tiny bit, just so it's snug on there again. Or try taking the tip of a pen or something and seeing if the actual connector on the modem wiggles.
