we lost cable for about 2 hours last night and it didn't receive it's nightly ping.

A DHCT will go into brick (----) for a few reasons:
1) Non-pay disconnect
2) remove of entitlements (account change)
3) loss of EMM's because it couldn't hear the forward data carrier for 20 or more days.

We try to "hit" every set top at least once every 30 days. With 170,000 of them on the plant, it would be impossible to do every night. A reboot typically will not fix whatever caused that box to lose it's authorization on the network.

If a set top was successfully booted and recieved it's entitlements, it theoretically can be "deaf" to the controller for up to and slightly beyond 20 days before it needs to be refreshed.

Why is that the case? if the account is kept up to date why does it have to be pinged?

Lots of reasons.

Non-payment
Code changes (addition/change of services in billing)
New channels and features
Quality control (missed codes in billing)
Sometimes you get a box that "walks" away from the head-end for whatever reason, and it needs to have it's privledges revoked.

I'm sure there are a few others that our billing system administrators could come up with, but those are the biggies. The boxes are also pretty autonomous if there is a serious failure, so keeping them up-to-date is just one more way they can continute to operate at a minimal level without help from the billing interface or the DNCS.

well, i don't know why we lost cable (and, speaking to someone on the phone is such a hassle - it sounds like were being pushed away from the phone)

but, now i know when this box get's it's pings or is it random?

Thats why i just let mine sit for a half hour unplugged hoping i wouldn't have to deal with them on the phone[always frustrating]
