Not very risky, not needed to be done-- though doing it is generally harmless enough. Once in a great while is plenty, if you do want to do it.

It's one of those things that one side will say is beneficial and the other side will say isn't.
Nobody seems to be able to prove conclusively, mathematically and/or technically, that there's any advantage, however-- other than saving a few MB of hard drive space.

I would say risk is low to moderate depending on your system and depending on which registry defrag tool you use. Some video drivers have been reported not working after a registry defrag.

Benefits range from none to minuscule. I recommend doing it once every five years.
