Got ahold of USR finally today. The fix for USR8200's that do not properly reset to factory:

Remove all net cables.

Hold reset button down and count to 5 1394 light flashes, then let go.
Once the power turns green, turn off unit for 5 minutes.

Turn unit back on, plug in cable from PC, renew DHCP on PC. Should reset.

USR tech says this is a rare issue - but I have had two already.
