I struggled with BellAtlantic (640/90) (Verison) from Feb 2000 till August 2000. I was down half of that time: Nightmare
I switched to Telocity (640/90)in August 2000. Very reliable connection (for the first three months). Static IP address! At $50 per month, not the best bargain around. During November and December 2000 Telocity had (still has) routing problems, email and newsgroup problems and shitty tech support. Dec 2000 Now for the good stuff.
Optonline ($29.00 per month) dynamic IP address (no servers here) 5000/1000 or close to it (Mororola Surfboard sb3100 modem).
The usual multi-tiered tech support is ok. The ool forum here is frequented by some capable ool staff that allow an alternative channel to resolve problems. This is a huge plus!
Six years later...still have OOL. I believe it costs a little more ($34.00) but the speead is now 15000/2000 and I have had a couple of different modems and now use the Motorolla sbv5120.
Speeds and quality of service are consistant and acceptable. The only outages have been for short periods of time (minutes rather than weeks of months that I suffered through with verizon).
